Once you have scheduled your appointment, please complete new patient paperwork and fax to the office or bring to your new patient appointment.

Dr. Barnett sees patients weekly at our Dallas location, twice a month at our Addison location.
Dr. Michael sees patients every Monday at our Addison Location and every Wednesday at our Dallas Location.
Dr. Bidiwala sees patients every Wednesday at our Addison location and every Friday at our Dallas location.
Please contact our office at 214-823-2052 to schedule all appointments at the office location most convenient for you. New patients may be requested to provide a medical history prior to scheduling their initial appointment.
Once you have scheduled your appointment, please submit your new patient information through our secure patient registration link located on the home page of our website as soon as possible. If you are unable to utilize this feature, you can download and print our new patient paperwork which is located under Patient Forms. We request that you have the paperwork completed prior to your appointment and arrive 15 minutes earlier than your scheduled time.
Please bring the following items with you to your appointment:
- Insurance card
- Driver’s license
- All MRI’s, CT’s/studies with radiology reports
- Any previous medical records that you feel might contain relevant information for our physicians
If you need to cancel an appointment, we kindly request that you notify us at least 24-48 hours in advance so that your appointment can be given to another patient in need. In the event that you are running late for your appointment, we ask that you please contact our office.
We make every effort to see our patients promptly, as we understand that your time is very important. However, sometimes emergencies arise in our busy neurosurgery practice involving urgent situations in the office or hospital to which our physicians must respond. In the event that this should occur, we appreciate your patience and understanding. If you are unable to wait, we will be more than happy to reschedule your appointment as quickly as possible.
