The physicians of Texas Neurosurgery, LLP has invested in the following facilities: Baylor Medical Center at Uptown and Methodist Hospital for Surgery.
These investments provide your physician an opportunity to be actively involved in the quality control over your medical procedures and to insure that your medical costs are reasonable. Your physician’s ownership interest in Methodist Hospital of Surgery and Baylor Medical Center at Uptown, does mean that your physician may benefit from choosing to perform surgical procedures on you at one of these facilities.
Because of this, your physician hereby advises you that you have the right to choose to be treated at some other facility at which they provide services. Your physician will make arrangements for such an alternative should you so desire.

Office Hours- Monday-Friday from 8:30am-5pm
Dr. Michael sees patients weekly at our Dallas and Addison locations and Waxahachie once per month.
Dr. Barnett sees patients weekly at our Dallas location, twice a month at our Addison location.
Dr. Bidiwala sees patients every Wednesday morning and every Friday at our Dallas location, and every Wednesday afternoon at our Addison location.
Please contact our office at 214-823-2052 to schedule all appointments at the office location most convenient to you. New patients may be requested to provide a medical history prior to scheduling their initial appointment.
Once you have scheduled your appointment, please submit your new patient information through our secure patient registration link located on the home page of our website as soon as possible. If you are unable to utilize this feature, you can download and print our new patient paperwork which is located under Patient Forms. We request that you have the paperwork completed prior to your appointment and arrive 15 minutes earlier than your scheduled time.
Please bring the following items with you to your appointment:
- Insurance card
- Driver’s license
- All MRI’s, CT’s/studies with radiology reports
- Any previous medical records that you feel might contain relevant information for our physicians
If you need to cancel an appointment, we kindly request that you notify us at least 24-48 hours in advance so that your appointment can be given to another patient in need. In the event that you are running late for your appointment, we ask that you please contact our office.
We make every effort to see our patients promptly, as we understand that your time is very important. However, sometimes emergencies arise in our busy neurosurgery practice involving urgent situations in the office or hospital to which our physicians must respond. In the event that this should occur, we appreciate your patience and understanding. If you are unable to wait, we will be more than happy to reschedule your appointment as quickly as possible.

Our practice continues to accept Medicare patients and accept assignment. We are more than happy to file our patients Medicare claims, as well as filing with any secondary payers. We also accept Workers' Compensation on a case-by-case basis.
In a true emergency, please go immediately to the nearest emergency room. If your urgent matter occurs after hours, please call our office at 214-823-2052 and leave a message with our answering service. The on-call physician will be notified and will return your call as soon as possible.
Texas Neurosurgery LLP accepts cash, checks, and all major credit cards. We offer convenient, personalized payment plans that automatically draft out of patients’ accounts.
Texas Neurosurgery LLP, provides our patients with office forms for your convenience. We prefer patients to complete and submit their new patient information online through our secure patient registration link.
If you are unable to submit these forms online, please print the new patient packet and complete prior to your appointment, making sure you have completed and signed every page, so your appointment time will not be delayed. We also recommend you arrive 20 to 30 minutes prior to your scheduled appointment, so your information can be entered into our system.

Texas Neurosurgery, LLP requires payment in full for any amounts that are the patient’s responsibility at the time services are rendered. This includes co-pays, co-insurance, and/or deductible amounts. Once your claim is processed by your insurance company, any additional amounts owed will be billed to you. If the patient’s estimated amount due results in an overpaid claim, then a refund will be processed once all claims are settled and there is no additional amounts owed by the patient.
You are responsible for knowing the specific rules of your insurance carrier. If your insurance carrier requires a referral, it is your responsibility to work with your primary care physician to obtain this referral prior to your scheduled appointment. If we do not have your referral number the day prior to your appointment, then you will be contacted to reschedule your appointment. If you are seen by one of our physicians without a valid referral, then all charges will be the responsibility of the patient.
Texas Neurosurgery, LLP does not accept Letters of Protection and we do not file claims with automotive insurance companies.
Failure to provide your current insurance information prior to services being rendered may result in denial of your claim. We assist our patient’s in receiving reimbursement from your insurance company, however please understand that you, the patient, have the final responsibility for your bill.
